Texas Residency Affirmation. If you are a Texas resident, this form is required to verify your state residency. The form must be completed in its entirety and returned to the Financial Aid Office.

2011-2012 Verification Worksheet. This form is required from all students who were selected for the verification process. Students should submit signed copies of their 2010 tax return and W2 forms. If the student is considered Dependent according to the 2011-2012 FAFSA, they will need to provide signed copies of their parents' 2010 tax return(s) and W2 forms also.

2011-2012 Unusual Circumstance Form. This form is for reporting unusual financial conditions for the 2011-2012 academic year. Financial conditions that may be used in an appeal include those that are both unusual (i.e. the typical family would not face this situation in a normal year) and non-discretionary (i.e., there's not much you can do about it.) Conditions that do not warrant an appeal would include payment of routine living expenses (house payment, car, credit card, etc.) debt repayment and other expenses that reflect lifestyle choices. Unusual circumstances may include something like unusual medical expenses, younger children in private schools in 2011-2012 or loss of income during the academic year 2011-2012.
